Welcome to ESF’s Online Enrollment Form! We are very excited that you will be joining us this summer. Our online enrollment form will take a few minutes for you to complete. Below are things you should have with you before you start:

• Contact Info – We will be asking for an emergency contact and anyone who will be allowed to pick-up your child(ren) at camp. Please make sure to have a phone number for those people you list.
• Friend Requests – If you will be making a request to group your camper(s) with a friend, please make sure to have the friend's first and last name.
• Checkbook or Credit Card – You are required to make a $300 deposit per camper or pay your full tuition by electronic check or credit card. ESF accepts VISA, Mastercard, and AMEX only.

HOW TO ENROLL
Complete the ESF Online Enrollment Form and make a $300 deposit per camper or pay your full tuition. Space is limited. Applications will be accepted on a first-come, first-served basis and enrollment is determined by the date and time your Online Enrollment Form is submitted. Once your Online Enrollment Form is received and it has been reviewed, a Confirmation Notice will be mailed to verify your child(ren)’s reservation or an Enrollment Coordinator will contact you. The balance of your tuition is due after you receive the Confirmation Notice.

FLEXIBLE SCHEDULING
We offer flexible scheduling: the weeks you choose do not need to be consecutive. You may also enroll for 2 or more programs at each ESF location (depending on age of camper). Please note: camp is closed on Wednesday, July 4, 2012.

FAMILY DISCOUNTS
A 5% Family Discount is given to all families enrolling 2 or more children. This discount applies to each child after the first in your immediate family. The first child, enrolled at the full tuition rate, is the camper with the higher tuition.

CANCELLATIONS AND CHANGES
If you need to cancel your child's enrollment, you must notify use in writing by April 1, 2012 in order to receive a refund. Cancellations on or prior to this date are subject to a $50 processing fee per child. No refunds will be given out after April 1, 2012 regardless of your registration date. There are no allowances given for late arrival, early dismissal, vacations, illness, or injury. Changes in the enrolled weeks are based upon availability and must also be made on or before the April 1, 2012 deadline.

PRIVACY POLICY
ESF recognizes that privacy is important. You will be asked to register for an ID and password and to provide certain personally identifying information (i.e. name, address, phone number, email address, birth date, ZIP code, gender, personal interests and credit card information for transactions. ESF uses your information to perform transactions, deliver services or information you have requested, and offer you products and services from ESF that we believe may be of interest to you. ESF also uses your information to contact you when necessary for customer service purposes.
